Book an appointment

Send a request below and the team phones or emails you back to arrange a time, or phone the rooms directly. Either way, a GP referral gives you a Medicare rebate.

Online

Request an appointment

This form is for appointment requests only. It is answered by the Complete Health Australia team on behalf of all four locations. They will phone or email you back during business hours to arrange a time. The form is not intended for clinical questions, results or prescriptions. Please phone the rooms about anything clinical, or raise it at your consultation.

Pick the prefix, then the eight digits that follow it. Typing or pasting the whole number works too and sorts itself out.

A current referral is needed for a Medicare rebate, and you can still request a time before it arrives. There is nothing to attach here: when the team contacts you they will tell you the easiest way to get the referral to them, or your GP can send it directly.

This helps the team explain the costs for any procedure. You do not need private cover to be seen, and consultations are not affected either way.

Please do not include medical details or symptoms. Those belong in the consultation, where they stay between you and your doctor.

Requests are not monitored for urgent problems. If you have severe abdominal pain, significant rectal bleeding, black stools or other urgent symptoms, contact your GP, attend an emergency department or call 000.

By phone

Rather talk to someone?

Each practice books its own appointments, so call the rooms where you would like to be seen.

Afterwards

What happens next

The team contacts you during business hours to settle a day, a time and which rooms. If your referral has not arrived yet they will tell you the easiest way to get it to them, and you will be told the consultation fee and the expected Medicare rebate before anything is confirmed. Referring doctors have their own page on how to refer, and anything clinical is handled by phone or at a consultation rather than online.
